Simply complete the Application Form and return it to us. If you
need any help completing this form, please call your selected FirstCaribbean Wealth Management Centre.
Step One
Complete all the details requested in Section A of the Application
Form as this information may be used for security purposes
once your account is opened.
Step Two
Decide which account(s) you would like to open and tick the relevant
boxes in Section B, remembering to sign the Customer Declarations.
Please ensure that you read our Terms and Conditions for
International Personal/Premier or
International Corporate as applicable.
After completing the form, please send all the relevant account
opening forms to your chosen FirstCaribbean Wealth Management
Centre. If you would like to apply for a FirstCaribbean
Gold Visa Card, please complete the enclosed application
form.
Please ensure that the following items are returned with your Application
Form:
1. A completed Banker’s
Confirmation Request Form for each party to the account;
2. A copy of each applicant’s passport, European Union identity
card, or USA or Canadian Driver’s Licence clearly showing
the photograph, signature, date of issue and expiry, and certified
with the following wording: “having seen the individual and
the identification document at the same time, I certify this is
a true copy and the photograph is a reasonable likeness”.
Certification may be by (in order of preference):
a) A branch of FirstCaribbean International Bank.
b) A branch of the Canadian Imperial Bank of Commerce.
c) Another international bank.
d) A consular or embassy official from your consulate or embassy.
e) A national or local government official.
f) A qualified lawyer who is a member of a national legal association.
g) A Notary Public situated in Canada, the USA or in the same country
as the branch of FirstCaribbean where you are applying to open your
account;
3. Either
(a) An original statement from a bank or building society, other
than the one you have provided details of on the Banker's Confirmation
Request Form, showing your residential address; or
(b) An original Visa, MasterCard, American Express or Diners Card
statement showing your residential address. The statement should
be no more than three months old. All original documents will be
returned to you; and
4. A completed and signed Consent
Form for each signatory. This form provides authorisation
to the Bank to provide information to governments under certain
circumstances, and must be completed.
After completing the Application Form, please send all relevant
account opening forms to your chosen FirstCaribbean Wealth Management Centre.
Please do not enclose cash or cheques for credit to your account
with your application. We will let you know when to send funds to
activate your account.
